From what I know; the Fallout 3 and Oblivion overworld were made mostly by procedural algorithm as a topological basis within the confines of predetermined parameters (map size, shape, elevation, and land flow). Then, artistic creation and detail was given to particular areas before being populated with objects, buildings, people, etc. This leaves you with some important, well-crafted places and some large areas that are mostly random geometry.
Skyrim was built entirely by hand, which is why it feels so much more lively and gorgeous. Fallout 4 will presumably continue this and is rumored to be 2 to 3 times the size of Skyrim.
